Two Part Confidentially Sign in Forms

Let your patients sign in without seeing the information from other patients. Your patients will sign in on the removable numbered label when entering your office and the info will copy down to the second sheet. The label strips will then be removed, and the patient information is now protected behind special backing paper. The first form, the original one; were your patient signed her name will be applied to the patients file folder or you could use it as a tracking system and call you patients by their numbers, so you won’t need to say their names aloud.
